This Consumer Health Data Privacy Notice (“Notice”) supplements the Premier Disability Privacy Policy, regarding our collection, processing, and use of “Consumer Health Data” (“CHD”) as defined under applicable state privacy laws.

We may collect CHD in order to provide certain products and services to you. Such CHD may include:

  • Information about your health conditions, treatment, or diagnosis that you provide to us in connection with your request for an attorney, where such information about health conditions, treatment, or diagnosis is related to your request for an attorney.
  • Any medical information or health records that you provide to us in connection with Premier Disability Services representation of your disability case.
  • Other CHD you provide to us in your communications to us.

We collect CHD from you as necessary to provide you with the products and services that you have requested.

CHD is shared with attorneys in our network for the purpose of providing you with the products and services requested by you. We may also share CHD with our subsidiaries and affiliates, in connection with your request for products and services.

We do not “sell” CHD as that term is defined by applicable state privacy laws.

Subject to applicable law, you have the following rights:

  • Withdraw consent to collect and share your CHD
  • Delete your CHD
  • Access your CHD
